General Conditions: 1. The appointments will be on adhoc basis, as per Government po icy. Applications will be addressed to the Principal Head of Institution, PMC, Allied / DHQ! General Hospitals Ghulam Muhammadabad & School of Nursing, Faisalabad. Last date of submission of applications is 09.03.2017. A set of photocopy of following documents will be required duly attested with the application:- 2. Matriculation Certificate or in ease of "0" Level equivalent certificate from inter Board Committee of Chairman, Islamabad showing total/obtained marks date of birth. Or school having certificate showing date of birth. 3. Intermediate Certificate! “A" level alongwith equivalent qualification from IBCC Islamabad showing total/ obtained marks. 4. Degree of MBBS and detailed marks of MBBS (11g to final profs) duly issued by Controller of Examinations showing total/ obtained marks and percentage and postgraduate qualification degree/ DMC in the subject. In case detailed marks are not produced then minimum passing marks will be considered. 5. Postgraduate qualification/ degree/ diploma in the subject. 6. Valid C.N.I.0 alongwrth Two passport size Photograph. 7. Valid PM&DC Registration Certificate, showing all medical degrees, postgraduate qualification therein. 8. Experience certificate must clearly the show the date of start and end of the prescribed job. 9. Domicile certificate of relevant district of province of Punjab issued on or before the closing date. Candidates presently leaving/residing abroad and having Punjab Domicik may also apply. 10. In case of Govt. servant, the departmental permission certificate duly singed and stamped by the competent authority of the concerned department. 11. No dues/No Inquiry/ Punishment awarded certificate must be attached. 12. Incomplete applications / only C.V. will not be entertained. 13. The number of posts may be increased and decreased. 14. The applications will be received during office hours. 15. Only eligible candidates will be called for interview provisionally after detailed scrutiny of applications of candidates. 16. No TA / DA will be permissible. NOTE: Experience certificate and foreign job must be recognized by PM&DC. Open experience certificate will not be accepted and considered incomplete. (IPL-1663)
